MEXICO CITY (AP) -- Mexico welcomed millions of children back to school Monday with masks, thermometers and globs of hand sanitizer, as scientists estimated the new strain of swine flu could have sickened 23,000 people before anyone realized it was an epidemic.

Key developments on swine flu outbreaks, according to the U.S. Centers for Disease Control and Prevention, the World Health Organization and government officials:
--Deaths: Global total of 61 -- 56 in Mexico; three in the U.S.; one in Canada and one in Costa Rica. One of those who died in the U.S. was a toddler from Mexico. Officials said the Canadian, U.S. and Costa Rican victims also had other medical conditions.

My patient was a 25-year-old man. He sat on the examination table, the picture of misery, coughing, red-eyed and shivering. His fever was 103. An interview and an examination suggested influenza (the rapid diagnostic test for flu wasn’t available at that time), but there was little I could offer him, other than ibuprofen and some homespun advice.
